What evidence do I need to register with Disability and Neuroinclusion?

When you register with us, you will be asked to provide evidence in order to support your request(s) for exam and/or learning adjustments. Depending on your specific needs or the adjustments you are requesting, we may need specific or additional evidence to what you may have already provided. A Disability Adviser can advise on the evidence we need and how you can obtain this. For a broad overview of the evidence we require relating to specific diagnoses, please see below.

Specific Learning Differences (SpLD)

We require a full diagnostic report confirming your SpLD. However, if you have previously received exam arrangements and have a JCQ Form 8 (which can be obtained from your school or college), please submit this to us via Surrey Support and select “Disability and Neuroinclusion” as your enquiry topic.so we can advise you on the most appropriate next steps.

If you already have a full diagnostic assessment report completed by a registered psychologist or a qualified specialist teacher holding a SpLD Assessment Practising Certificate, please contact the MySurrey Hive team via Surrey Support,

Autism

We require a letter from your GP or another appropriately qualified specialist confirming your diagnosis and outlining its impact on your day-to-day activities and studies. Alternatively, if you had an Education, Health and Care Plan (EHCP) issued by a Local Authority during school or college, we can accept this as evidence.

If you do not currently have evidence of a diagnosis, please refer to the section on undiagnosed disabilities for further guidance.

Attention Deficit Hyperactivity Disorder (ADHD)

We require a letter from your GP or another appropriately qualified specialist confirming your diagnosis and outlining its impact on your day-to-day activities and studies. If you do not currently have evidence of a diagnosis, please refer to the section on undiagnosed disabilities for further guidance.

Mental health conditions

We require a letter from your GP, care coordinator, or another appropriately qualified mental health or medical professional. This should confirm your diagnosis and outline the impact your mental health has on your day-to-day life and your ability to engage with your studies.
If you are finding it difficult to obtain this evidence, or are unsure what documentation to provide, please raise a case with the MySurrey Hive team via Surrey Support, selecting “Disability and Neuroinclusion” as your enquiry topic.

Mobility difficulties

A letter from your GP or another appropriately qualified specialist can be very helpful in ensuring we provide appropriate support and make the right adjustments.

Sensory impairments

Generally, a letter from your GP confirming your condition will be sufficient. We can also accept letters from appropriately qualified specialists. The more detailed information your GP or specialist can provide—particularly regarding any specific support you may need—the more helpful this will be.
Please note that appointment letters are unlikely to be accepted as appropriate evidence.
