How can I change doctors?

If you need to change doctors and register with a new GP surgery, you can do this online via the NHS App or by visiting your new GP surgery in person. 

If you visit the surgery, you will usually be asked to complete a registration form, and the practice will arrange for your medical records to be transferred.
